Vax Nevada Days Official Rules
As part of a public health initiative, the Nevada Office of the Governor and Department of Health and Human Services is partnering with Immunize Nevada and IGT Global Solutions Corporation (IGT) (the “Program”) to launch “Vax Nevada Days” to encourage Nevadans to receive the COVID-19 vaccine (the “Project”).
Each week of Vax Nevada Days the Project will conduct random drawings to identify vaccinated Nevada residents as winners of cash, post-secondary education saving plans, state parks annual permits or fishing licenses (the “Promotion”). Winners will be announced weekly starting July 8, 2021, with the winner of the GRAND PRIZE OF $1,000,000 announced August 26, 2021. Vaccinated Nevada residents will have nearly 2,000 opportunities to win.
Eligibility for the vaccine promotion is limited to Nevada residents who have received at least one dose of a recommended COVID-19 vaccination at any time prior to the end of this promotion.
By claiming a prize or otherwise participating in the Promotion, you agree to abide by and be bound by these Rules.
To be eligible to claim a Prize in this Promotion, you must meet all of the following criteria:

- You are at least 18 years of age for cash prizes, or at least 12 years of age with consent of a parent or guardian for 529 savings plans and non-cash prizes.
- You have received at least one dose of a recommended COVID-19 vaccination in Nevada at any time prior to the end of this Promotion.
- You have signed and submitted all required documentation and releases as part of the claims process by the required deadlines, including without limitation, proof of identity and consent to publicity.
The following persons are NOT eligible to claim a Prize in this Promotion:

A “drawing” is a single drawing within the Promotion.
The Promotion consists of multiple weekly drawings for various prizes, plus one Grand Prize Drawing for the Grand Prize of $1 million. See the Drawing Schedule below.
A winner of a weekly drawing remains in the pool for future drawings. The sooner you receive the vaccine, the more opportunities you have to win!

Winners will be randomly selected from a list of Nevada residents who have received at least one dose of a recommended COVID-19 vaccination that was recorded in the Nevada WebIZ system (“Record”). A unique number will be assigned to each Record by the State Immunization Program (“NSIP”) and only those numbers will be provided to IGT, which will utilize computerized tools to randomly select the winners. No personal identifying information will be provided to IGT.
Winners and Alternates
In addition to the winner, alternates will also be drawn for each drawing. The NSIP will review the eligibility of each winner. The Program will contact the winner to further verify the winner’s identity and eligibility. If the Program is unable to contact the winner, unable to verify the winner’s identity or eligibility, or if the winner refuses the prize, the Program will repeat the process with the first alternate, and, if necessary, any successive alternate winner(s) until an eligible winner can be verified.
CLAIMING A PRIZE:
Notification and Eligibility Verification
The Program will attempt to contact each winner following a drawing. If the Program cannot reach a winner within 48 hours from the time of the Program’s Initial Contact, that winner is deemed to have forfeited the prize and the Program will award the prize to the next alternate winner. “Initial Contact” means the date and time an employee of the Program attempts to contact a person regardless of whether the contact is successful.
All potential winners are required to submit additional information to verify eligibility and identity to claim the prize. This may include, but is not limited to, proof of Nevada residency, age, occupation or employer, and contact information.
Winners ages 18 and over must agree to the release of their identity to the public for promotional purposes. Parents or legal guardians of minors must agree to the release of the minor’s first name to the public for promotional purposes. The Program may release the winner’s name, city or county, prize amount or type, and vaccination status to the media and the public.
Receipt of Prizes
No prize will be delivered until the winner’s identity and eligibility has been verified. Delivery time will vary depending on the type and size of the prize, and the Program does not guarantee the delivery of any prize by any specific date.
Waiver and Release: By claiming a prize or otherwise participating in, or attempting to participate in, the Promotion, you agree that you will not assert any right, claim, or cause of action against the State of Nevada or any of its officers, agencies, employees, or affiliates, including without limitation Immunize Nevada, as well as any technology partners that have assisted the State of Nevada in the drawings, or any of its officers, employees, affiliates, or agents, on account of any disqualification, determination of ineligibility, failure to contact you as a potential winner, or any other act or omission taken in the course of administering the Promotion. You hereby forever waive and disclaim any and all rights or claims you may or could have, known or unknown, arising in any way from the Promotion.
The winners of the Prizes will be randomly selected from among eligible vaccinated Nevadans whose vaccination information has been entered into the Nevada WebIZ immunization system before the drawing. Individuals who do not meet the eligibility requirements defined in these official rules are not eligible to win.
If a winner cannot be reached for any reason within 48 hours after the Initial Contact of the winner following the drawing, that winner is deemed to have forfeited the prize. The method of attempting to contact potential winners, using the contact information available, is within the sole discretion of the Program and is not subject to appeal or review.
If a winner refuses a prize, or fails or refuses to provide any information necessary to verify his or her eligibility or identity, or refuses to agree to the public release of their identity for promotional purposes, the winner forfeits the prize.
If a winner dies or is for any other reason unable to claim the prize, the winner shall be deemed ineligible.
In any case where a winner has forfeited the prize, is ineligible, cannot be contacted, or otherwise cannot receive the prize, the Program will award the prize to an alternate. Alternates will be used in the order in which they were selected (i.e. alternate one (1) will be used first, alternate two (2) second and so on) as needed. Any prize that cannot be awarded by the end of the promotional period may be reverted back to the Program.
A decision to replace a winner with an alternate for any reason is within the sole discretion and judgment of the Program. After the Program decides to proceed with an alternate, the person who was previously selected as a winner is no longer entitled to the prize.
The probability of winning any prize will be determined by the number of vaccinated Nevadans who are eligible for the Promotion.
Drawing dates and prizes are subject to change or modification as determined by the Program. The State of Nevada reserves the right to end the Promotion prior to all prize winners being drawn.
The Program will make all determinations concerning the validity of entries, winners, and other decisions required under any drawing or promotion. The decision of the Program shall be final and is not subject to any review or appeal.
The Program reserves the right to reschedule any times and dates regarding any aspect of any drawing or promotion without advance notice. The Program reserves the right to void any drawing without advance notice and to conduct a new drawing if the Program, in its sole discretion, determines it is necessary to maintain the integrity of the drawing.
